On the cast boolits web site there is a person who makes the 3 piece set you are looking for and sell them for just under $30, I bought one from him a few years back. On EBAY they sell for at least twice that.

Mike,
 Hornady made a pistol powder measure that used/made the powder bushing you are talking about, it is 1/2 inch tall, I don't think they make the bushing any more. I have their 1986 catalog with a powder chart, PM me a email address and I will send you a picture of it. It shows #5 bushing to be 3.7 grains of bullseye. Bruce in NJ who owns Star makes the bushings.

I am the third owner of my Star Universal 45ACP which I purchased in the early 1980s. When it came to me the owner claimed it to have loaded approx 1 million rounds. A few years after I got it there started to be issues with skipped primers, I finally looked down the primer tube with light on the other end and could see wear in the tube. Replaced the tube at least 20 years ago and it has been fine since. You may have a worn spot in your primer magazine tube that is allowing a primer to flip from time to time. I also own a Star Universal (4 years) in 38 spec and the primer magazine tube has had primers(Winchester)  get stuck in it a few times now.

 Star tool head with 38 special perfection dies, shell plate, 3 grain bullseye 00 powder slide, powder slide cover and base, small primer punch and bushing, primer pick up tube, primer magazine tube, shell floor plate bushing. Everything needed to convert a Star universal to 38 special. $325 shipped. I could not figure out how to post pictures so send a message and I will email them.
 I purchased a Star Universal set up for 45acp and 38 special, these are the parts I do not need.(already have a universal in 38 spec)
